Step 1: Submit the Sign-in Sheets to CME Dept Sign-in Sheets Activity Planner 2 weeks after the Activity
Step 2:   CME Dept grants credits based on Attendance/Sign-in Sheets Evaluation Forms (Session/Speaker Evaluation, Pre and Post Surveys, and Activity Evaluation) CME Dept 4 weeks after the activity
Step 3: CME Dept sends email to attendees with instructions on claiming their certificates CME Dept 4 weeks after the activity
Step 4:
  1. Submit Evaluation Summary Reports
    • Speaker Valuation by Learners
    • Event Evaluation by Learners
    • Pre/Post Survey Gap Analysis Report
  2. Submit copies of Syllabus/ Handouts
  Activity Chair/ Activity Planner 2 months after the activity
Step 5: Submit Final Income/Expense Report Budget Worksheet (Ask CME Dept for templates) Activity Chair/ Activity Planner 2 months after the activity
Step 6: Consider a 3-month Post-Activity Survey to assess impact on practice/ practice change (If applicable) Post-Activity Survey Activity Chair 3 months after the activity
Step 7: Submit Activity Improvement Plan

  2. Final Budget Form with revenue and expenses
