If you think your study meets the criteria for expedited review, please submit the following documents (via email and interoffice mail) to the Office of Research Administration (ORA):
1. MCH Application for Research
2. MCH IRB Submission Form
3. Protocol
4. Related documents such as questionnaires, surveys,
etc., if applicable
5. Informed Consent Form, if applicable,
or appropriate HIPAA form
6. Recruitment materials (flyers, ads, etc.), if applicable
7. Copy of grant or contract, if study is externally funded
8. Copy of CV for all investigators*
9. Copy of updated human subjects protection
training certificate*
*if not already on file with the ORA.
